Tandym Group is Hiring an Admissions Nurse Near New York, NY

A healthcare network in New York City is looking to add licensed Registered Nurse (RN) to their admissions team in Manhattan. In this role, the Hospice Nurse (RN) will be responsible for conducting assessments to admit patients to their hospice program.

About the Opportunity:

  • Specialty: Hospice
  • Patient Population: Adults / Geriatrics
  • Schedule: Monday to Friday
  • Hours: Standard business


Responsibilities:

The Admissions Nurse (RN) will:

  • Perform physical and psychosocial assessments of potential hospice patients in order to develop individualized plans of care. Assessments take place in both private homes and facilities.
  • Admit eligible patients into the hospice program following appropriate standards of practice established by the National Hospice and Palliative Care Organization (NHPCO).
  • Perform other duties, as needed


Qualifications:

  • 2 years in med-surg, community health, oncology or hospice.
  • Graduate of accredited School of Nursing
  • NYS Registered Nurse (RN) license


Desired Skills:

  • BSN
  • Previous hospice experience
  • Previous intake or admissions experience.
