Description:

Responsible for coordination of care for the residents living at Water’s Edge Care Center. Consult with staff, physicians, and management team on nursing problems and interpretation of facility policies to ensure resident needs are met. Monitors staff activities and care provided to seek out opportunities to enhance the quality of care provided to the resident with an emphasis on person-centered care.

Job Functions:

    • Initiates and implements the policies and procedures that govern the Nursing Services in collaboration with the interdisciplinary team.
    • Assist in the completion of needed assessment for admission, review the 48-hour CP and make changes if needed, assist in the completion of discharge paperwork, medication reconciliation, and identify potential discharge needs in collaboration with therapy/social services.
    • Participates in care conferences by contributing information, keeping the Supervisor informed of changes, Documentation weekly summaries that accurately describe the resident
    • Audits the charts and computerized records for completeness and timely notification to family and Physician.
    • Initiates, reviews, and updates the 24-hour report sheet. Emphasis is placed on follow-through and consistent updating and completion of the 24-hour report, as it is carried over from day to day.
    • Rounds on the Unit are completed daily with “Just in Time” training completed as issues arise.
    • Evaluate, assess, and reassess, based on the change of condition report and complete the required documentation. Uses and understands the SBAR and Interact Critical pathways to ensure standards are followed.
    • Consult other departments, as appropriate, to provide for an interdisciplinary approach to the resident’s needs. If faxing an MD is required the Registered Nurse will monitor that the response has been obtained promptly and the resident’s need has been addressed as well as Family has been notified
    • Treats residents and their families with respect and dignity. Identifies and addresses the psychosocial, cultural, ethnic, and religious/ spiritual needs of residents and their families.
    • Ability to adequately assess pain. Utilizes appropriate pain management techniques. Educates the resident and family regarding pain management. Notifies MD of need for order changes timely, monitors that staff is following through, and evaluates the effectiveness of the pain relief measure.
    • Demonstrates the knowledge of the geriatric resident and evaluates licensed nurse’s abilities to assess and reassess residents. Completes required competencies timely, educates staff when it is needed
    • Keeps the Director of Nursing, his/her representative, and/or the Administrator informed regarding circumstances or situations, that have or may have a serious impact: including daily observation/knowledge and those identified during the survey process
    • Assist other staff nurses by being a resource, by kindly providing education and coaching, assisting them with Physician rounding and orders, teaching and promoting facility Goals and events by being a role model
    • Ability to perform point-of-care testing per Clinical Laboratory and interpret results of waived tests and procedures; takes appropriate action on waived test results.
    • Following the five- (7) mediation rights reduces the potential for medication errors provides education to the nursing staff if an error should occur and documents that in-service on the incident report.
    • Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors.
    • Supervises Licensed Practical nurses and nursing assistants in their daily work. Responsible for assessments of lungs, cardiac, and changes in mental status assessments for COC for the LPN. Assists in education and orientation of new employees and evaluations. Follows procedure for call-in, by attempting to find replacement or delegating to LPN for the notification of infection control of employee illness.
    • Reviews and evaluates all incident reports for completeness and assists in determining the Root cause by following the 5 Whys. Assuring that documentation is completed using critical thinking skills
    • Coordinates and directs resident care by reviewing the resident’s care plan and observing for proper implementation of the care plan by the nursing staff for that unit. Makes changes to the Plan of Care as needed and provides information on changes to other staff members
    • Monitors the clinical record and computerized charting for follow through and accuracy of medical administration and that the evaluation of effectiveness is documented about Pain, Behavior, Fluid, Weights, Bowel elimination status, etc.
    • Conducts unit meetings and or shift-to-shift meetings regularly with a focus on process improvement
    • Exhibits genuine interest in and appreciation of adults in the later years of life and is an advocate for the residents of the facility
    • Completes staff rounding for safety, team building, managing up, and process improvement of facility goals. Records results of rounding/ implements interventions for safety concerns turns reports in timely to D.O.N
    • Is a motivated, independent individual who can organize workload so that all functions are completed timely
    • Works on maintaining a good rapport and cooperative working relationship with the Director of Nursing, physicians, other departments, and staff.
    • Complete charts and care audits as a way to identify compliance with Regulations/State & Federal
    • Assist the Director of Nursing in identifying, implementing, and evaluating processes for improvement
    • Knows solid food and liquid temperatures. (Solid food should be no lower than 140°F and no higher than 170°F. Liquid is recommended to be no lower than 120°F and no higher than 170°F.)
    • Receives annual blood glucose meter training procedure
    • Takes the responsibility to be a team leader by correcting those behaviors that are not STAR standards
