Duration : 8 weeks

Hours : 40hrs / week

Shift Timing : Day, 9AM - 5PM

License and certifications :

• Active New York State License as Registered Nurse
• BLS (AHA)

Responsibilities :

• Assess and evaluate members in MLTC and MAP lines of business, utilizing UAS information.
• Foster collaboration with PCPs, engaging in discussions regarding member care.
• Review assessment findings with PCPs to uncover any overlooked concerns.
• Identify member risk factors and categorize accordingly.
• Prior approve additional services based on assessments and evidence-based standards, referring complex cases to the Medical Director.
• Contribute to team care planning meetings.
• Address and resolve complaints promptly, assist in grievances, and support Quality Management Department requests.

Experience :

Two (2) to three (3) years clinical experience in certified home health agency (CHHA), Lombardi program and / or MLTC or MAP program.

Education :

Bachelor’s Degree required

BSN preferred.
