We are looking for an individual with a passion for helping others to be part of our Home Health team. The Registered Nurse will be responsible for working directly with patients and their families, performing comprehensive home assessment and ensuring that patients are receiving the best possible care at home. Our Registered Nurse is responsible for providing ongoing support to patients and educating them about the importance of taking their medications as prescribed. Our Registered Nurse will recognize the challenges of many of our patients and provide excellent outcomes to ensure that they are receiving the care they need.

Responsibilities:

• Accurately perform health assessments and document the results, such as vital signs, wound, or skin conditions.
• Follow a treatment plan while providing support and educating patients and their family members.
• Administer medicines and treatments according to the treatment plan and work with other medical professionals as needed.
• Identify changes in the patient's condition and inform the patient's primary care doctor or on-site physician of any significant changes.
• Accurately communicate with other medical professionals regarding the health of the patient.
• Provide comfort to patients.

Pay: $40.00 - $50.00 per hour
