Client Care Coordinator Responsibilities:

a.Conduct client introductions and train andsupport caregiverteammembers 

b.Workin conjunctionwiththeOfficeManagertocoordinateon training schedule

c.Assist with covering shifts in the event of call-offs

d.Assist with filling shifts

e.Actaskeycaregiverwhennotconductingtraining

f.Identifykeyareasoftrainingandset upappropriatedeliverysystemsforcaregivers including caregiver orientation

g.ConductQualityAssessment atclient sitesandonthephone

h.Act asambassadoron behalfofthe organization

I. Helptoimproveemployeeandclientretentionratesthroughactivecommunication and problem-solving efforts

L. Providing exemplary customer service

M. Communicating regularly with clients and their families

N. Other duties as assigned

O. Evaluate care notes and ensure training compliance amongst caregiver team

Client Care Coordinator Requirements:

• Experience – Two years supervisory or management experience in a home care setting preferred; Training experience in a caregiving environment
• Education – High school diploma and CNA preferred
• Skills – Developing personal relationships, time management skills, exemplary communication skills
• Characteristics – This is an autonomous position, so we’re looking for individuals that are self-motivated and willing to learn.
