Major Responsibilities

  • This position is responsible for the referral management and coordination of patient admissions within the Central Admissions Office (CAO).
  • Manages eReferral platforms (may include Allscripts/Careport, NaviHealth, Ensocare and others) used by assigned referral source(s). This includes meeting response time goal of less than 20 minutes, ensuring center(s) profile information is up-to-date, bed availability is received each day, and the overall effective management of assigned eReferral platform.
  • Completes referral evaluations for potential referrals/inquiries/admissions to identify their respective clinical and financial needs.
  • Assists Case Managers and care center sales and marketing teams in selecting post-acute placement.
  • Utilizes CRM to track all accepted referrals and begin admission process
  • Obtains all appropriate clinical documentation from the hospital to support services and/or reimbursement level. All documentation will be uploaded into Point Click Care (PCC) for center.
  • Verifies all insurance information to ensure potential admission has a valid payer source.
  • Provide innovative, responsible healthcare with the creation and implementation of new ideas and concepts that continually improve systems and processes to achieve superior results.

Minimum Qualifications

  • Minimum of two years' experience in healthcare setting working with patient admissions in a sales and marketing capacity or clinical capacity.
  • Experience in a post-acute healthcare setting preferred.
  • Experience reviewing clinical documentation and financial documentation.
