Job Details:
- Conduct patient intake and assessment to identify emotional, social, and environmental barriers related to their treatment and/or life situation.
- Ability to diagnose independently using DSM-5-TR.
- Develop treatment plans to address mental, emotional, behavioral, developmental, and addictive disorders, impairments, and disabilities, reactions to illnesses, injuries, disabilities and social problems.
- Provide person-centered psychotherapy, both short- and long-term to individuals, couples, families and groups.
- The ability to safely manage patients in crisis.
- Facilitate the care plan across a multidisciplinary care continuum, which may include interacting with consulting physicians, other health care providers, billing and/or third-party payers.
- Familiarity with local mental health and case management resources to make referrals as appropriate.
- Timely documentation in the medical record in accordance with department policies and OMH guidelines.
- Maintain patient confidentiality.
- Maintain a working knowledge of relevant medical/legal issues that impact patient care.
- Perform other duties as assigned.
- Master's Degree and NYS license to practice social work (LMSW) from a confidential, physical location in the State of New York.
- Excellent organizational and interpersonal skills.
- Ability to engage patients.
- Ability to work both collaboratively with others as well as independently.
- Bilingual preferred
- $65-$70K annually, commensurate with experience.
- Clinical Supervision towards LCSW licensure.
