We are looking for an experienced Case Manager to join our team. The successful candidate will be responsible for providing comprehensive case management services to individuals and families in need. This includes assessing needs, developing service plans, coordinating services, monitoring progress, and advocating for clients. The ideal candidate should have excellent communication and problem-solving skills, as well as a strong commitment to helping others.

Responsibilities:

• Assess and evaluate clients' needs and develop appropriate service plans.
• Provide case management services to clients in need of assistance.
• Maintain accurate and up-to-date records of client information and progress.
• Develop and maintain relationships with community resources to ensure clients have access to necessary services.
• Provide crisis intervention and support services as needed.
• Develop and implement strategies to ensure clients are receiving the most effective services possible.
• Coordinate with other service providers to ensure continuity of care.

Job Type: Full-time

Pay: $24.00 - $32.00 per hour
