Job Title: Weekend Overnight Program Monitor - Veterans Commons - Overnight
Location: Veterans Commons, 150 Otis St, San Francisco, CA 94103
Employment Type: Part-Time, Non-Exempt
Schedule: Saturday & Sunday; 10:00 PM to 6:00 AM
Summary
Swords to Plowshares seeks Program Monitors to provide services to disabled veteran residents at our Veterans Commons residence. The Program Monitor supports the achievement of veteran resident goals, case management, and veteran advocacy with the goal of keeping the community safe. The responsibilities of this role include monitoring the activities or residents, responding to emergencies, and completing appropriate documentation, including incident reports and logbook entries, also monitoring meals, and supporting clinical staff.
Veterans Commons is a permanent supportive housing residence for 75 chronically homeless and disabled veterans. Swords to Plowshares developed the Veterans Commons at a surplus city property with the help of many community partners and agencies. This is a city and VA funded project and will be staffed 24/7 by clinical and program team employees. Program Monitors will report to both the Property Manager and Program Manager. The Program Monitor is a non-exempt position.
Roles and Responsibilities
  • Maintain good attendance record, including timely notification if unable to work shift
  • Monitor activities of residents to insure resident and facility safety
  • Document incidents
  • Follow appropriate response procedures
  • Provide services within a Harm Reduction framework
  • Communicate effectively with treatment and property management staff to ensure the well-being of veteran residents
  • Coordinate with clinical staff to stabilize veteran residents and to maximize their potential for success
  • Respond to emergencies including incident reports, electronic progress notes, logbook entries, and the notification of supervisory staff
  • Ability to drive clients to appointments as needed, and/or provide pick up delivery service as needed
  • Monitor activities of residents after business hours
  • Perform other required duties per supervisory staff
Requirements
Experience
  • Minimum of three (3) years' experience working with "hard to serve", substance abuse and mental health clients, with at least one (1) year in a human service agency
  • Translatable military experience and skills may be alternatively substituted
Qualifications
  • Applicants with de-escalation and conflict resolution training recognized
  • An understanding of and experience working with issues of homelessness and recovery issues
  • Employee must be able to perform all essential job requirements and responsibilities within what would be considered reasonable accommodation
  • Must be able to communicate effectively with a variety of personalities and be comfortable working with an at-risk client population, including embracing the agency's Diversity, Equity, and Inclusion initiative

  • Ability to remain calm and responsible during emergencies
