POSITION SUMMARY:
Assists the care coordination team (provider, nurse, case managers, etc.) by providing medical social work services to patients and their families to enhance psychosocial functioning and continuity of care. Provides advocacy, information and referral services to patients and families to address their medical and psychosocial needs. Completes appropriate documentation including nature of discharge needs, resources available, patient and family response and discharge plans arranged. Provides resource information to patients and families as appropriate for future care needs. Participates as key member of care coordination rounds.
EDUCATION/TRAINING:
LSW with either a Bachelor’s degree or a Master’s degree or a LCSW with a Master’s degree. (LSW must be obtained within 6 months of assuming the position).
LICENSURE/CERTIFICATION:
Current license (LCSW) required within 6 months of assuming the position or certification from the State of Illinois required. Bi-Annual BLS certification required.
(Current staff as of 03/08/18 – will need to obtain BLS certification by 09/30/18)
EXPERIENCE/SKILLS/ABILITIES:
Successful completion of annual clinical job specific competencies and skill verification tools required. Knowledge of principles of growth and development over the life span. Good interpersonal skills and ability to work with patients, physicians, families, managers and members of the community under varied conditions. Excellent organizational, critical-thinking and conflict resolution skills required.
