Pay: $35-$50/hour Weekend Shift Differential

• Weekend Shift Differential: $1.00 per hour additional on weekends.

Nurse - RN Overview:

As a Nurse - RN at Elmore Assisted Living, you'll spearhead the coordination and planning of residents' care, ensuring that our care practices prioritize the well-being of each individual. Your responsibilities will include:

• Comprehensive Care Oversight: Oversee and coordinate the care plans of our residents, ensuring that they reflect each resident's unique needs and preferences.
• Clinical Excellence: Conduct thorough clinical assessments of residents' health statuses, meticulously documenting all pertinent information.
• Chronic Illness Management: Monitor and manage the status of chronic illnesses, overseeing the administration of medications with precision and care.
• Collaborative Care: Foster seamless collaboration with external healthcare providers and internal team members to optimize resident care outcomes.
• Family Engagement: Interact with residents' families and loved ones, providing regular updates and reporting any changes in their health or care plans.

Nurse - RN Qualifications:

• State Licensure: Licensed as a Registered Nurse / RN in the state of Minnesota.
• EMR Proficiency: Experience with Electronic Medical Records (EMR) software, ensuring accurate and efficient documentation.
• Geriatric Experience: Prior nursing experience in skilled nursing facilities (SNFs), assisted living (AL) communities, transitional care units (TCUs), or home health settings is highly desirable.
• Autonomous Work Ethic: Demonstrated ability to work autonomously, making informed decisions and prioritizing tasks effectively.
• Exceptional Skills: Possess excellent planning, organizational, and communication skills, essential for delivering top-tier care.
