JOB DESCRIPTION

Promotes and restores patients’ health by maintaining the day-to-day operation of the patient care area; assisting support/Medical Assistant/Provider staff; working with the clinic provider to assure physical and psychological support of patients at the level for which the CNA/LNA was trained and certified. Will coordinate care for patients on the provider’s daily schedule daily schedule and proactively manage and coordinate care for patients not on the schedule. Work in collaboration with Medical Assistants/LPN/RN to ensure clinic is operating at the highest level of performance.

ESSENTIAL FUNCTIONS

  • Projecting empathy, professionalism, and a positive image of SLH through excellent communication and customer service skills, to support patients/families, facility personnel and SLH processes to ensure an excellent experience.
  • The LNA and CNA provides or assists in the delivery of nursing or nursing-related services under the supervision and direction of a licensed nursing staff member. A licensed nursing staff member may be a Licensed Practical Nurse (LPN) or a Registered Nurse (RN).
  • The CNA/LNA possesses the knowledge, skills, and the attitude to effectively meet standards for competent professional practice according to the Arizona State Board of Nursing scope of practice.
  • The organizational behavioral competencies which reflect organizational mission, vision, and values.
  • The CNA/LNA may perform the following functions (these are not all inclusive):
    • Taking vital signs, height, and weight using standing and wheelchair scales
    • Observing and reporting pain, documenting in the patient record
    • Assisting with diagnostic tests including obtaining specimens.
    • Recognizing and reporting abnormal patient physical, psychological, or mental changes to the provider or nurse.
    • Obtain and document in the patient record, information relevant to the patient visit for the provider
    • Apply clean bandages/wraps/splints, assist with procedures, and exams as instructed.
    • Assisting in admitting (rooming), transferring (emergency transfers), or discharging (end of visit communication/referral information/follow up/education as directed) patients
    • Perform required infection control measures i.e., cleaning rooms, machines, etc. in preparation for patients
    • Safety and emergency procedures, including cardiopulmonary resuscitation
    • Health care team member skills including the role of the nursing assistant and others on the health care team, time management and prioritizing work
    • Other functions as defined in rule R4-19-802 (F. 1.-14.) (https://www.azbn.gov/sites/default/files/2020-03/RULES.Effective.June3_.2019.pdf) and other duties as assigned.
  • The CNA/LNA demonstrates competency within the domains of Continuous Quality Improvement, Patient and Family Centered Care, Professionalism, Safety, and Teamwork.
  • Obtains phone messages off the in-office voice mail, for MA/RN/LPN to prioritize. Urgent/emergent telephone messages are addressed first according to direction from prioritizing staff.
  • Frequently reviews EHR communications for telephone encounters, documents, and other messages. Calls patients to relay provider derived information.
  • Tasks learned through in-service or educational training if the nursing assistant has demonstrated competence performing the task
    • The task CNA be safely performed according to clear, exact, and unchanging directions.
    • The task poses minimal risk to the patient or resident and the consequences of performing the task improperly are not life-threatening or irreversible.
    • The results of the task are reasonably predictable; and
    • Assessment, interpretation, or decision-making is not required during the performance or at the completion of the task.
  • The CNA/LNA CAN not administer medications including but not limited to injections, p.o. medications, topicals, inhalants, etc.
  • The CNA/LNA may administer oxygen in an emergency or Dr. Stat situation, after completion of the oxygen related competency.
  • Interact with patients and employees in a respectful manner consistent with the mission and values of Sun Life.
  • Review and follow up on patient self-management goals. Assist in providing patient education and counseling as directed by the Provider/RN/LPN.
  • Review preventative care needs of the patient and alert the provider such as male and female preventative exams, immunizations, and other recommended age-related screenings.
  • Ensures pre-visit information is reviewed and completed
  • Maintain documentation of patient care services by completing patient and department records in accordance with established policies and procedures.
  • Document procedures, delivery of care, and pertinent information regarding care rendered to the patient. Routine and timely documentation of all core process work activities including but not limited to, chart reviews, patient calls, and correspondence
  • Answer telephone calls and take messages for the provider and or return telephone calls to patients, documenting all patient interaction in the patient medical record
  • Contact patients to report lab test results, change in medication
  • Contact patient by phone or letter of any no show/missed appointments
  • Facilitates any patient reports and deliver’s them to the provider. Provide an effective communication link between patient and medical staff, including relaying messages from providers
  • Act as a liaison with the providers, other doctor’s offices, hospitals, sites of patient care
  • Protect patients and employees by complying with infection control policies and protocols, following medication administration, storage procedures and controlled substance regulations
  • Maintain Universal precautions and follow all applicable SLFHC procedures regarding infection control and reducing healthcare acquired infections (HAIs). This includes but is not limited to, good hand-washing and gloving practices
  • Cleans and prepares rooms between procedures, and restock supplies
  • Completes patient care requirements by performing tests and completing other tasks as assigned, i.e., set up rooms, place patient in room, takes patient’s vital signs, height, weight, pulse ox, oxygen levels, etc. and records them in the chart, prepares patient for examination by the provider, assists with performing EKG’s on patient, and assists the provider with patient’s care
  • Performs specimen collection and Performs laboratory CLIA Waived testing for which they have received training
  • Assures quality of care by maintaining clinic standards of care while adhering to requirements of other governing agencies
  • Ensures operation of medical equipment by completing preventive maintenance requirements; following manufacturer’s instructions; reporting malfunctions, requesting repairs, maintaining equipment inventories, and evaluating new equipment and techniques
  • Maintains nursing supplies inventory by anticipating needed supplies. Keeps rooms stocked with adequate medical supplies, ensuring that all medical supplies items are within the expiree date
  • Reports communicable disease to County Health Department as needed
  • Adheres to expectations of required/mandatory in-services and certifications.

Qualifications


EDUCATION AND EXPERIENCE

  • CNA/LNA active and in good standing with the Arizona State Board of Nursing
  • One year of CNA/LNA experience adult and or pediatric settings preferred
  • Medical Terminology class with passing rate of 80% or higher preferred, class can be taken within 90 days of hire
  • Current Cardiopulmonary Resuscitation (CPR) Health Care Provider Level

COMPENSATION

Salary: $16.50 - $20.00

***Compensation is dependent on the relevant year and months of experience***
