Statement of Purpose: This role and title are inspired by the work done by the World Health Organization (WHO) related to Healthy aging and functional ability. They state the healthy aging process includes "creating the environments and opportunities that enable people to be and do what they value throughout their lives. Everybody can experience healthy aging. Being free of disease or infirmity is not a requirement for healthy aging, as many older adults have one or more health conditions that, when well controlled, have little influence on their well-being. Their healthy aging definition is "the process of developing and maintaining the functional ability that enables wellbeing in older age. Functional ability is about having the capabilities that enable all people to be and do what they have reason to value. This includes a person's ability to: meet their basic needs; learn, grow, and make decisions; be mobile; build and maintain relationships; and contribute to society. Functional ability consists of the intrinsic capacity of the individual, relevant environmental characteristics, and the interaction between them".

This is a varied position with responsibility for the successful implementation and outreach of the Health Aging and Functional Ability Program to optimize the function, health, safety, and well-being of the Sun Health residents and Sun Health at Home members. Must have a strong desire to serve the elderly and a passion for individuals living with dementia. This program is supported by person-centered care, the Allen Cognitive Levels and assessments, and the Dementia Capable Care training and model.

Some of the main responsibilities:

  • Establish, direct, and review applicable written policies and procedures that support the specialized program's mission statement and goals.
  • Coordinate and align care programs (nursing, ALs, activity, therapy, dining) and required documentation that supports the specialized program's mission statement and goals.
  • Assure that personnel, residents, families, visitors, and attending physicians follow established policies and procedures.
  • Recruit, screen, and train care program leadership personnel in cooperation with appropriate department heads and under facility and program policies and
  • In coordination with Nurse and Life Enrichment Manager(s) ensure all identified care staff have the competence and confidence to perform their job responsibilities related to this specialized program.
  • Be available to mentor and model delivery of Healthy Ageing and Functional Ability techniques and Dementia Capable Care under all policies and procedures and assist staff with problem resolution.
  • Facilitate and lead or co-lead Healthy Ageing and Functional Ability and Dementia Capable Care Committee/QI meetings, family educational forums/support groups, and participate in resident/family council meetings when requested.
  • Provide consultation and resource information for persons requesting information or tours.
  • Provide/facilitate educational programs for community organizations, professional organizations, and related healthcare persons, schools, etc. in support of the Sun Health mission.
  • Meet with families and social service representatives during inquiries about the specialized program offerings. If not available, follow up with a phone call to the family.
  • Ensure pre-admission assessments are completed according to each level of living. Coordinate and review the pre-admission information and assessments with appropriate team members before admission and assist in the determination of the appropriate level of care and level of living.
  • Ensure Household Managers and Clinical Honse Managers follow up on all memory care facility residents hospitalized for more than 24 hours by visiting the hospital or supplying dementia-specific caregiving information to the hospital staff. Follow up with family and provide progress reports to the program staff. Be available and support the program participant's families and significant others as partners in
  • Monitor all member assessments and assist with conversations around the best options to support healthy aging which may include moving to another level of living within Sun Health.
  • Work closely with all facility directors/managers to deliver and promote an exceptional interdisciplinary specialized program.
  • Coordination with staff scheduler and managers, responsible for staff scheduling to reflect the life story of the residents and level of care needs.
  • Develop, implement, and monitor quality improvement/assurance measures to ensure program accountability and participate in regularly held meetings.
  • Maintain awareness of best practices and research advancements and integrate as appropriate in specialized healthy aging and dementia programs and teachings.
  • Promote research programs focused on enhancing the resident's abilities and improving caregiver
  • Work closely with grant implementations and fund-raising programs directly affecting the specialized dementia program.

Qualifications:

  • Minimum of a Bachelor's or Master's, in Occupational Therapy, Social Services, Gerontology, Nursing, Recreational Therapy, Doctorate of Occupational Therapy preferred.
  • At least 3 years of leadership and experience in dementia care. Must have managerial/supervisory experience along with presentation skills experience.
  • Experience working in Senior Living or Healthcare is a plus.
