About the team:

The Licensed Mental Health Professional (LMHP) will provide counseling services with the support of a multi-disciplinary team to include primary care and substance use disorder treatment in a Rural Healthcare Clinic. The LMHP will conduct biopsychosocial assessments, diagnostics using the DSM-5, individualized treatment plans, therapy, and counseling for a diverse caseload including adults, adolescents, and families.

Required Education and Experience:

  • Must have a National Provider Identifier (NPI) required.
  • Two years of recent clinical experience preferred.
  • Experience working with families and children preferred.
  • Experience working with clients with co-occurring disorders who exhibit current or history of substance use disorder preferred.

Required Licenses, Certifications and/or Registration:

  1. LICSW (Licensed Independent Clinical Social Worker) in the state of Washington, or
  2. LMFT (Licensed Marriage and Family Therapist) in the state of Washington, or
  3. LMHC (Licensed Mental Health Counselor)
  4. Associate licenses will be considered.
  5. Current Basic Life Support (BLS) certification required.

Job Specific Duties:

  1. Performs bio-psycho-social intake assessments, diagnoses using DSM-5 criteria, develops individualized treatment plans, and conducts therapy using evidence-based practices.
  2. Collects data from a variety of sources such as patient records, interviews, observation, and team members. Documents patient assessment and progress appropriately; maintains timely, complete and accurate records.
  3. Works as a member of a multi-disciplinary team. Contributes to team-based care through effective partnerships with patients, their caregivers, and other providers. Follows up on referrals in a timely manner with treatment recommendations. Communicates issues, variances and problems to other members of the care team in an appropriate and timely manner.
  4. Meets face-to-face with patient for immediate crisis intervention, behavioral health intervention or other social service related issues. Participates and monitors the appropriate tracking system for identified patients. Coaches patients/families toward successful self-management.
  5. Collaborates with internal and external resources to facilitate a safe transition from hospital to another facility or home with appropriate services. Identifies barriers and initiates action to resolve.
  6. Serves as the point-of-contact, advocate, and informational resource for assigned patients, family, care team, payers and community resources. Proactively acts as patient advocate; responding with empathy and respect to resolve patient/family concerns.
  7. Builds and maintains effective working relationships. Serves as a liaison across disciplines. Participates as an active member of committees; leads work groups, projects and committees as assigned.
  8. Maintains professional skills and awareness of regulatory changes, compliance issues, innovations and developments, through research and training/continuing education opportunities. Stays abreast of trends and best practices.
  9. Performs special projects and other related duties as assigned.

The pay range for this position is $69,502.09 to $102,542.13 per hour, depending on experience
