Summit Medical Consultants is a rapidly growing Physician Practice. Founded in 2015, we have expanded to approximately 100 employees and are continuing to grow. Our mission is to provide patients and their families with robust clinical services throughout their Acute Inpatient Rehab, Skilled Nursing, Long Term Care and Assisted and Independent Living stays. We coordinate with Post Acute Providers, Specialists, Surgeons, Physical, Occupational, and Speech Therapies, Case Management and Psychiatry to provide compassionate and coordinated care.
DESCRIPTION
Under supervision, the Licensed Practical Nurse acting as a Care Management Coordinator will perform a variety of medical and clinical support functions in the long term care/skilled nursing setting to provide support to our triage nursing team, providers, facility staff and administrative team.
Job responsibilities include, but are not limited to:
Relaying provider given verbal orders through EHR to facility nursing staff
Assisting with entering medication orders
Performing venipuncture and/or lab draws
IV insertion as licensure and certification allows
Documentation of and assisting with provider-directed care plan
Attending IDT/care conference meetings
Reviewing and documenting patient medical history in EHR system
Assisting in paperwork for discharge planning with facility staff
Managing incoming fax box and routing appropriately
Completing documentation in EHR
Managing patient records for our practice
Assisting with any incoming medical records/authorization requests
Making Home visits to patients as necessary
Assisting patient with provider directed DME orders for equipment
Coordinating care with outpatient specialties
Coordinating a 7 day follow up visit via Telemedicine with Summit providers
Completing various 30 day follow up calls
