Summit Home Care is Hiring a RN SUPERVISOR Near Middlesex, NJ
Summit Home Care is a family-owned and operated home care agency. We are currently looking for compassionate and reliable Case Managers to join our team of dedicated professionals. We are currently servicing Monmouth and Ocean County and taking on new clients every day.
Responsibilities:
- Works with Patient Coordinator to ensure accurate clinical documentation for case opening
- Opens cases and initiates the plan of care, medical orders, MAR, and TAR and revises as necessary
- Conducts monthly client visits in-home and virtually as needed
- Initiatives in-home supervision Q 60 Days and or as needed
- Participates in formal and informal problem-solving processes to meet the EMR computer education needs and established outcomes for users
- Is the liaison between Summit and community, clients, facilities, etc.
- Evaluates the outcome of care while providing support to ensure the services being rendered are meeting the client’s need
- Informs the physician and other staff of changes in the client’s needs.
Requirements:
- Current RN license in the state
- 2 years experience supervising
- Previous Home Care experience
- Knowledgeable with NJ DOH regulations and policies
- Up to date first aid and CPR certification
- Exceptional staff training and customer service skills
- Compassionate and friendly demeanor
