Summit Community Care Clinic is Hiring a Family Physician Near Frisco, CO

Position SummaryThe Summit Community Care Clinic (SCCC) Staff Physician is responsible for the delivery of professional medical services, and supervision of staff, residents, and students. The Staff Physician plays an active role in Quality Improvement activities within the clinic.
The Physician must demonstrate regard for the rights of all patients and provides respectful and considerate care focused on the patient's individual needs. The Physician will provide a safe and confidential environment for patients, families, and personnel.
Essential Functions
  • Promotes the philosophy and mission of Summit Community Care Clinic.
  • Demonstrate accountability and responsibility for duties and tasks associated with the position.
  • Acquire, maintain, and demonstrate appropriate knowledge levels and competencies.
  • Act as a representative of the clinic with patients, staff, and the general public in a professional and supportive manner.
  • Provides direct patient care following established protocols, including but not limited to conducting outpatient clinic; establishing patient history through interview and physical exam; establishing diagnostic impression; managing chronic health problems; initiating arrangements for treatment- including diagnostic laboratory, imaging, and other modalities. Makes decisions concerning medical care and the needs of the patient, as well as decisions regarding special needs, following established protocols.
  • The delivery of professional primary care services on behalf of SCCC, within the scope of training and expertise of the Employee. Hours and number of patients scheduled will be mutually determined by Employee and SCCC.
  • Participation in the Quality Assurance Program of SCCC, including attendance at QA Committee meetings and Peer Review processes, as appropriate.
  • Actively participates as a member of the Integrated Care team. Works directly and through referrals with the behavioral health and dental team to evaluate/treat integrated care needs. Refers and collaborates with community agencies as appropriate.
  • Ensures appropriate documentation and maintenance of medical records within the electronic health record including patient visit notes, timely test follow-up communication, and accurate coding practices.
  • Assists with clinical supervision of advanced practice providers.
  • Performs other related job duties as assigned by the Chief Medical Officer.
  • Analyzes reports and findings of tests and examinations, and diagnoses condition of patient.
  • Administers or prescribes treatments including accurate medication prescribing involving the patient in shared treatment decisions.
  • Prescribes and administers vaccinations to immunize patients from communicable diseases.
  • Promotes health by promoting preventative measures of care including screening, nutrition, and physical and mental health in accordance with professional guidelines.
  • Provides outpatient obstetric and pediatric care within the scope of practice.
  • Performs surgical procedures commensurate with surgical competency.
  • Refers patients to specialty-level care when necessary.
  • Ensures transitions of care are completed and documented within the electronic health record.
Additional Requirements
  • Grants are a vital part of Care Clinic revenues and directly or indirectly support all positions. This role may have responsibilities that are closely associated with grant deliverables. Grants and grant responsibilities may change over time.
  • Complete all required training as designated by HR and the position supervisor.
  • Adhere to all policies within the organization and those outlined within the SCCC Employee Handbook.
  • Comply with all SCCC Vaccine Mandates.
  • Willingness to travel and work at other SCCC locations as determined by the supervisor.
  • Comply with all applicable Credentialing and Privileging requirements timely.
  • Embrace the SCCC Mission and Vision and agree to work within the SCCC Guiding Principles and the principles set by the position supervisor.
  • Complete additional duties as assigned by your supervisor.
  • Must participate in quality assurance/ improvement activities when applicable.

Education/ExperienceMust Haves to be Successful:
  • Completion of a residency program in Family Medicine or Internal Medicine.
  • Current Colorado M.D. or D.O. license (or ability to apply immediately upon offer).
  • Current DEA licensure required (or ability to apply immediately upon offer).
  • 3 years minimum experience as an M.D. or D.O. in a primary care setting or equivalent experience in residency and training.
  • Interpersonal skills and sound judgment are necessary to communicate with a wide variety of persons, including the indigent, and to work with the Clinic staff in providing the medical services of the Clinic.
  • Exhibit cultural competence and exceptional teamwork skills.
  • Ability to read, write, and clearly speak the English language.
  • Ability to interpret a variety of instructions and deal with multiple linguistic and cultural variables.
  • Ability to relate cooperatively and constructively with patients, co-workers, administration, physicians and providers, community agencies, referral sources, regulators, and other health team members.
  • Ability to enlist the cooperation of others.
  • High tolerance for stress.
  • Excellent organizational and leadership abilities.
  • Outstanding communication and people skills.
  • Knowledge of the industry’s legal rules and guidelines.
