Summary:
The VP for Integrated Care Management is responsible for building and leading high performing, closely integrated, provider based Care Management teams and activities across multiple clinical sites within Summa Health System with the goal of transforming healthcare and delivering high quality, efficient patient care.

The VP is responsible to work with the SVP to drive the Health System’s strategy related to coordination of care across the continuum, with emphasis on ambulatory and community based care, acute care management and utilization management, as well as the transitional interfaces with acute care and post acute settings. In collaboration with SVP and New Health Collaborative leadership, the VP provides leadership in the prioritization, development, implementation and evaluation of care management programs and transitional care activities that affect population health, patients' experiences of care and per capita cost reduction. He/she will maintain close monitoring of outcomes and performance data, and be responsible for adjusting program design and implementation to achieve program goals. The VP will continue to innovate and build programs and collaborations that enhance Summa Health System’s ability to manage medical expense by leading efforts to develop targeted programs that impact the highest cost patients, such as clinical specialty programs (i.e. palliative care), and other community collaborations that strengthen connections across the continuum of care.The VP will have a key leadership role in shaping population health and program vision, recruitment and retention of high-performing care management teams, and will play a critical role in interfacing with clinical sites to optimize integration of care management and transitional care teams into existing clinical operations.
Minimum Qualifications:
1. Formal Education Required:

  • Master’s Degree in Nursing or related field
  • Current Registered Nurse (RN) license eligible for practice in the state of OH
2. Experience and Training Required:
  • Extensive experience in clinical operations in a hospital, health system, provider practice, and/or ambulatory health center
  • Experience building and leading population health initiatives
  • Experience building and leading a large team from the ground up
  • 10 years of combined experience in care management delivery in multiple care settings (e.g., inpatient, ambulatory, remote, health plan, etc.)
  • Minimum 7 years of related management experience
  • Proven leadership experience building relationships through support and encouragement with multi-disciplinary teams, physicians, and other healthcare providers
  • Knowledge of federal and state regulatory requirements and accreditation standards
  • Process Improvement experience or training
3. Other Skills, Competencies and Qualifications:
  • Population Specific Competency: Ability to effectively interact with patients/customers with the understanding of their needs for self-respect and dignity
  • Ability to partner with a variety of clinical sites where care management programs are deployed
  • Ability to work as a liaison between all healthcare delivery services to ensure continuity and coordination of care processes
  • Advanced analytical abilities, including ability to define gaps and opportunities for improved performance and translate performance insights into operational actions
  • Possesses strong writing, organization, and facilitation skills
4. Level of Physical Demands:
  • Sedentary: exerts up to 10 pounds of force occasionally and/or a negligible amount of force frequently.
