The VP is responsible to work with the SVP to drive the Health System’s strategy related to coordination of care across the continuum, with emphasis on ambulatory and community based care, acute care management and utilization management, as well as the transitional interfaces with acute care and post acute settings. In collaboration with SVP and New Health Collaborative leadership, the VP provides leadership in the prioritization, development, implementation and evaluation of care management programs and transitional care activities that affect population health, patients' experiences of care and per capita cost reduction. He/she will maintain close monitoring of outcomes and performance data, and be responsible for adjusting program design and implementation to achieve program goals. The VP will continue to innovate and build programs and collaborations that enhance Summa Health System’s ability to manage medical expense by leading efforts to develop targeted programs that impact the highest cost patients, such as clinical specialty programs (i.e. palliative care), and other community collaborations that strengthen connections across the continuum of care.The VP will have a key leadership role in shaping population health and program vision, recruitment and retention of high-performing care management teams, and will play a critical role in interfacing with clinical sites to optimize integration of care management and transitional care teams into existing clinical operations.
