Summary:
Utilizes the nursing process and evidence-based practice to provide patient-driven, holistic care with the collaboration of the Core Team (Patient, Primary Care Physician, RN Care Manager, Therapy and aide) and Expanded Team (family/caregiver and internal and community-based services involved in providing care to the patient). Develops care protocols that support care for a panel of patients throughout the continuum of care to assure that care is timely, appropriate, of high quality and cost effective. Utilizes health education and telecommunication as applicable to promote therapeutic relationships and shared decision-making between the patient and the interdisciplinary team to support patient self-management, prevention and wellness. Demonstrates leadership in the delivery and improving holistic care through collaborative strategies with others. Employs performance improvement strategies to reach quality and cost effectiveness goals.
Minimum Qualifications:
Formal Education Requirements:
  • Graduate of an accredited school of nursing
  • Current registered nursing licensure in the State of Ohio
  • Baccalaureate degree required
Experience and Training:
  • Five years medical-surgical experience or equivalent required
  • Hospice and palliative certification desired or to be obtained within 2 years of employment
  • One year home care/hospice experience/case management experience or previous experience with population health or disease management program experience required
  • Experience working with families with chronic illness or palliative preferred
  • Staff education experience preferred
  • Knowledgeable of applicable regulations and standards surrounding telehealth

  • Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds
  • Strong organizational skills, provide log of completed daily tasks.
  • Ability to work well under pressure with a strong attention to detail
  • Ability to work in a complex institutional structure requiring significant collaboration
  • Demonstrates basic skills as well as navigation and application of electronic devices and platforms
  • Maintains confidentiality of all patient information
  • Requires interpersonal skills sufficient to interact effectively with patients, their families, interdisciplinary caregivers, peers, subordinates, and supervisors.
  • Current CPR required
Level of Physical Demands:
Office Sedentary: Exerts up to 10 lbs of force occasionally and/or a negligible amount of force frequently.

$33.66/hr - $50.49/hr

The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials. Many factors, such as years of relevant experience and geographical location are considered when determining the starting rate of pay. We believe in the importance of pay equity and consider internal equity of our current team members when determining offers. Please keep in mind that the range that is listed is the full base salary range. Hiring at the maximum of the range would not be typical.
