Stratford Pines Nursing & Rehabilitation is Hiring a Social Services Director Near Midland, MI
Do you have a passion for caring for others? Do you have a desire to help others achieve their fullest potential? We are looking for a motivated, energetic, and caring licensed Social Worker.
We are seeking a Full Time Director of Social Services responsible for providing the services necessary to meet resident and family social and emotional needs and participating in the development of their comprehensive care plans.
Summary of duties:
- Assessing and completing the assigned sections of the (MDS) Minimum Data Set & Comprehensive Care Plan.
- Develop, Coordinate and Promote social interaction, reality orientation and intellectual stimulation.
- Conduct daily resident rounds
- Act as a liaison with social, health and community agencies.
- Assess, plan and document residents' discharge needs
- Coordinate family and community resources as needed to solve financial needs and promote emotional security.
Wage:
- Based upon licensing and experience.
- Non Licensed: $20.85/hour-$22.35/hour
- BSW: $25.35/hour
- **Including $0.85/hour supplemental pay through 09.30.2024
Qualifications:
- A bachelors degree in Social work OR...
- A bachelors degree in a human services including but not limited to sociology, special education, rehabilitation counseling, and psychology and one year of supervised social work experience in a health case setting working directly with elderly individuals.
- Strong Interpersonal and time management skills
