Company

Stokes Counseling Services is a leader of mental health private practice, and group practice within the state of Connecticut. We are a group practice made up of licensed clinicians (LPC, LMFT, LCSW, LPCA, LMSW, MFTA) who provide an array of services including individual, family, and group counseling.

The WISE Program services Elders, Medically Compromised, and Disabled Communities (including families and caregivers). This is an outpatient Mental Health level of care program (we are NOT a crisis program, Clinicians are NOT on call). This is an opportunity to work a Hybrid Model where you work Telehealth AND do in-home work.

Responsibilities

  • Develop service or treatment plans based on assessment of strengths, needs, collaboration with referral agencies, and collaboration with the individual served.
  • Provide activity and participation-based interventions and approaches developed from training materials, experience, and individual and/or families' successes.
  • Make referrals to services and resources on behalf of individuals and/or families served.
  • Assure that documentation and records are kept confidential, organized, and submitted to the supervisor in an accurate and timely manner.
  • Participate in team meetings with peers and collateral team meetings when needed.
  • Support peer development by addressing areas of concern for professional development and team performance issues.
  • Provide an aftercare plan for each family/individual served.
  • Build and maintain positive relationships with community partners from referring/ collateral agencies. Guide the family in their interaction with referring agencies, and other community organizations as needed.

Qualifications

  • We are looking for clinicians who are passionate about providing treatment for individuals and families
  • Valid Driver’s License is REQUIRED and must provide own transportation 
  • Must be licensed to practice in CT and a resident of CT or one of the following states, NY, NJ, MA, or NC
