POSITION DESCRIPTION

JOB DESCRIPTION: Intake Coordinator (40 hours per week)

IMMEDIATE SUPERVISOR: Admission Manager

POSITION SUPERVISED: Intake Department

QUALIFICATIONS/CREDIENTIALS:

1) Microsoft office competency required.
2) Experience in electronic medical records systems.
3) Excellent oral and written interpersonal communication skills;
4) Experience in record management.
5) Experience in working in managed care environment.
6) Excellent organizational skills.
7) Basic knowledge of the DSM5 and ASAM criteria.
8) LCDCIII, LISW, LSW, or LICDC licensure and at least one year experience in the field of substance abuse treatment and disorders.
9) Must pass a background check and complete a drug urine screen.
10) Fluency in both English and Spanish language is not required, but is preferred.

BASIC FUNCTION:

The Intake coordinator is responsible for all aspects of intake and admissions to clinical and housing programs at Stella Maris including forms, initial assessments, appropriate program placement, entry counseling, and client orientation.

ESSENTIAL JOB DUTIES AND RESPONSIBLITIES:

1) Demonstrates knowledge of agency program services, goals and treatment philosophy.

2) Manage and complete timely admissions into all levels of care on referrals.
3) Attend team meetings and collaborate with team members to maintain a professional and proficient work environment.
4) Network with outside referral agencies to assist in the admission process and gather recent and correct information on clients.
5) Demonstrate an understanding of ASAM criteria requirements for admissions into each level of care and refer clients appropriately based on their needs.
6) Communicate effectively with fellow counselors and nurses on the intake team to ensure client safety and medical appropriateness for specific levels of care.

7) Complete initial assessments on clients to determine their care needs before they enter SUD treatment.

8) Abide by the rules and regulations listed in the Health Insurance Portability and
Accountability Act.

9) Administer urine drug screens to clients entering care and accurately read and record the results.

10) Enter demographic information, assessments, consent forms, urine screens, and any other necessary client information into the electronic medical records database in a timely manner.

11) Search client belongings upon entering to treatment and dispose properly of contraband items.

12) Demonstrates knowledge of current assessment, diagnosis and treatment models.

13) Demonstrates knowledge of DSM-V diagnostic criteria and ensuing placement criteria

14) Demonstrates knowledge of the Ohio MITS system and is able to navigate other managed healthcare computer sites to determine clients placement and appropriate funding source.

15) Demonstrates thorough knowledge of community resources for recovering persons and based upon that knowledge is capable of making appropriate referrals.

16) Demonstrates the ability to effectively de-escalate client crisis situations.
17) Maintains statistical records pertaining to the intake admission department.
18) Demonstrates a sensitivity to cultural diversity issues and compliance with confidentiality and clients rights issues.
19) Demonstrates the ability to interact with clients and external professionals in an appropriate manner.
