Requirements:

• Active License
• BLS
• COVID Card

Experience: 12 months of experience required in past 3 years

Duties:

• Provide direct patient care in accordance with the hospice plan of care
• Assess patient needs and develop individualized care plans
• Administer medications and treatments as prescribed by the physician
• Monitor and manage symptoms such as pain, nausea, and respiratory distress
• Assist with activities of daily living, including bathing, dressing, and feeding
• Educate patients and families on end-of-life care options and support services
• Collaborate with interdisciplinary team members to ensure comprehensive care
• Document patient assessments, interventions, and outcomes accurately and timely
