This position is located at the Department of Mental Health, Inpatient Services Forensics Department, 7901 Farrow Road, Columbia, SC 29203. As the Social Worker, working under limited supervision, performs diversified social work duties for a complex population consisting of clients with multiple diagnoses and psycho-social problems including criminal charges. Maintains a caseload of patient's requiring intensive psychiatric treatment by completing all documentation and clinical services related to the caseload assigned. Participates in discharge planning activities. Performs case coordination for multi-disciplinary treatment team process. Responsible for preparation of thorough and accurate Psychosocial Assessments and Forensic Review Board Reports for assigned patients recommended for discharge. As directed, supports Forensic Outpatient Social Work initiatives. Responsibilities include but are not limited to: Maintains a caseload of patients requiring intensive psychiatric treatment by completing all documentation and clinical services related to the caseload assigned. Participates in discharge planning activities and assumes the major role for implementation of referrals, scheduling of appointments and the sharing of recommendations with community agencies and programs to ensure continuity of care. Performs case coordination for multi-disciplinary treatment team process. Coordinates with treatment team, family, patients and other collateral sources, monitors patient's treatment and provides information for treatment team to ensure patients' needs are being met. Prepares thorough and accurate Psychosocial Assessments and Forensic Review Board Reports for assigned patients recommended for discharge. Attends core competency training and all required trainings as assigned. Attends monthly Social Work meetings and maintains Social Work Licensure. Maintains knowledge and enforces policies and procedures.After at least one (1) year of experience with the agency, will be able to serve as field instructor or preceptor for students completing their field placement at Bryan Psychiatric Hospital.
EXAMPLE OF DUTIES
State Minimum Requirements: A bachelor's degree in social work from a program accredited by a nationally recognized accrediting body for social work programs, or from a social work program whose standards are at least equivalent to the minimum standards required by the nationally recognized accrediting body approved by the Board.Agency Minimum Requirements: A master's degree from a social work program accredited by the Council of Social Work Education. LMSW-SC Board of Social Work Examiner's and one (1) year of experience at this level.Note: If any Agency Requirements are listed above, applicants must also meet those requirements to be considered for the position.

The selected candidate must have basic computer/technology skills to complete essential job duties. Evaluations are conducted in a secure area and monitored by Public Safety. Must have a thorough knowledge of social principles, practices & techniques as, as well as general knowledge of Federal and State laws pertaining to social services programs & CST/CR issues. Must be capable of delivering Social Work services to a diverse and highly vulnerable population that presents with multiple psycho-social problems with minimal supervision of the technical aspects of Social Work.
