Core Responsibilities:

The community-based Intensive Case Manager(s) will demonstrate expertise in working with both mental health and substance abuse treatment programs, will have knowledge of services and providers of mental health services and eligibility requirements, and will have knowledge of the necessities and challenges to successful community living. They will have comfort with proactive outreach to key stakeholders to develop regional resource familiarity and develop additional resources in anticipation of needed services. Finally, they will demonstrate expertise in assertive service provision with an eye toward diversion from criminal justice involvement and/or other disruptions to successful community living.

Job Duties will include but may not be limited to:

The Community-based Intensive Case Manager(s) will:

·Provide professional services work in intensive case management for clients in the community who are at-risk of disrupted services or at-risk of involvement in the Criminal Justice System.

·Advocate for and/or assist clients in gaining access to and maintain medical, psychiatric, self-help, rehabilitation, social, educational, psychological, residential, financial, and other necessary support services.

·Appropriately confer with clients, professional medical/clinical personnel, providers, and other concerned parties in order to identify unmet needs and necessary supports/services and develop a plan of assistance based on clients’ needs, preferences, circumstances, and team assessments. 

·Assist clients in accessing and maintaining needed services in order to support clients’ community integration efforts and plan.

·Proactively outreach to key stakeholders to increase familiarity and anticipate needed services.

·Document all client-related interactions and be able to provide data as requested regarding all services rendered.

·Work is performed under limited supervision and regional/state-wide travel including occasional overnight travel may be required.

Minimum Qualifications:

An Associates Degree in one of the Social or Behavioral Sciences and 4 years’ experience in professional level mental health work -OR- a Bachelors Degree in one of the Social or Behavioral Sciences and two (2) years’ experience in professional level mental health work -OR- a Master’s Degree in one of the Social or Behavioral Sciences.Equivalent work experience in professional level mental health work may be substituted for education on a year-for-year basis.

Candidates must have or be eligible for a Mental Health Rehabilitation Community Certification (MHRT/C).

Preference will be given to candidates who have experience working with justice involved clients.

We partner with providers, communities, and citizens to offer support, resources, education, training, technical assistance, and services. 

This position is located in Houlton and will require travel throughout Aroostook County.
