Human Services Analyst

  • Serves as Behavioral Health Referral Coordinator for the Health Care for the Homeless Program with responsibility to:
  • Arrange referrals for behavioral health care services.
  • Ensure timely and accurate collection and input of patient data.
  • Monitor patient referrals for behavioral health care services and follow up on services that are overdue as appropriate.
  • Provide behavioral health coordination services to referred patients in a timely manner by submitting referrals on a daily basis to the Provider.
  • Coordinate patient follow up appointments as needed.
  • Understand patient behavioral health care needs and effectively address those needs.
  • Foster effective community partnerships to meet the behavioral health care needs of the patients.
  • Serve as point of contact for behavioral health care partners.
  • Copy, scan, and fax medical records and other documents.
  • Maintain confidentiality of all records, patient interactions, and services according to HIPAA privacy policies
  • Serves as backup to the Medical Referral Coordinator, with responsibility to:
    • Print records from HMS and scan into NextGen
    • Submit referral requests to Specialty Care department for authorization or extension
    • Contact patients when referral is authorized or denied
  • Perform other related duties as assigned including emergency duty

Minimum-

  • Access to reliable transportation
  • Experience working in a medical clinic environment, including knowledge of medical terminology
  • Three or more years of customer service experience
  • One of more years of behavioral health experience

Preferred-

  • Three or more years of data entry experience
  • Experience working with the homeless population
