The Case Manager is responsible for ongoing needs assessment, personal goal plans, implementation, support, information, referral, and advocacy for individuals who are in the Supportive Housing program.
This position requires the ability to connect with clients who have experienced long-term homelessness, have mental and chemical health concerns, as well as physical health problems as a result of their life on the street.
This position will influence and recognize positive behavior changes and will identify problems and obstacles that threaten to undermine the client’s self-sufficiency.

Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job. While performing the duties of this job, the employee is regularly required to sit, climb stairs, drive, bend, and communicate. The employee must lift and/or move up to 15-30 pounds. This job involves flexibility in working in an office environment as well as traveling daily to clients' homes.
