SUMMARY
Acts as the primary point of coordination for eligible individuals. SSA’s assess the needs and wants of individuals on caseload, develop the individual service plan (ISP), and authorize and monitor the services to ensure individuals are living a good life.
This SSA must be fluent in American Sign Language (ASL), have an understanding of deaf culture, and will have a caseload of predominantly hearing impaired individuals.

An integral part of the job function is ensuring that services, supports and oversight of such supports occurs with the focus on the individual receiving services. Person-centered planning is the foundation for all services, supports, monitoring, supervision and coordination for the individuals served. Some positions within the SSA department are not assigned a caseload and complete specific functions as noted above (75%).

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Must have a valid Ohio Driver’s License, an acceptable annual BMV Check, evidence of a good driving record, and proof of insurance. Fluency in ASL is required.
EDUCATION AND EXPERIENCE
Bachelor’s Degree in related field. Must be fluent in American Sign Language. Experience with individuals who have developmental disabilities, and eligible for DODD Certification.
