Team Leader (Lead Worker/ Supervisor) job announcement. Star Choices Inc. is a nonprofit organization that provides person centered, community based supports to people with disabilities. We are currently seeking qualified applicants for the position of Lead Worker/Supervisor in the Hall County, Flowery Branch, GA area.

GENERAL STATEMENT OF DUTIES :

o Supervision of Direct Support Professionals (Staff that work directly with individuals whom experience disabilities)

o Supervision of supports and services of individuals that experience disabilities that are supported in group residences, Host homes, and in personal residence.

o Providing 24/7 phone emergency assistance and if needed physical assistance on a rotation with 2-4 other Team Leaders.

o Providing initial/ ongoing training with assistance from Program Coordinator to new/ current staff.

o Hiring, developing, and firing of staff.

o Making 2x Monthly visits to each person supported.

o Assure Direct Support Professionals are implementing policies and procedures appropriately, by random checks-in's, announced/unannounced visits, etc.

o Working directly with Individuals that experience disabilities. If staff are unable to perform Direct Service Duties then Supervisor finds coverage or provides direct coverage as emergency fill in.

o Provide scheduling for Direct Support Professionals

o Updating and making changes to schedule weekly to accommodate family request, employees changes in working availability, staffing availability etc.

o Track documentation needed to bill Medicaid for reimbursement.

o Reviewing documentation as needed for quality and content.

o Code billing each month for all individuals supported.

o Monitoring documentation of support provided to the individuals we support.

o Monthly compliance expectations.

o Tracking important dates for Individuals Service Plans (ISP) and other required dates.

MINIMUM QUALIFICATIONS

Organizational skills-

o This supervisory role will require organization of many objectives and interest at once. This role will require meeting needs in individuals we support, the schedules and needs of individuals that work for the agency, and working families of individuals we support. Supervisor will be expected to set meetings, dates, etc. to meet above objectives. Supervisor's will need to have good organizational skills as much of the work will be done in individuals homes and in the community not in a traditional office building.

Decision Making:

o Specific goals and obstacles to achieving those goals, generates alternatives, considers risk, and evaluates and chooses the best alternative in order to make a determination, draw conclusions, or solve a problem.

Customer Service:

o Works with families to assess needs, provide assistance, resolve problems, and satisfy expectations, knows individuals and services, and is committed to providing quality service to individuals we support.

Problem Solving:

o Identifies problems, determines relevance of information, and uses sound judgment to generate and evaluate alternatives and to make decision about courses of action.

Working Environment:

o Subject to varying and unpredictable situations. Subject to many interruptions. Travel will be regular with this position. The individuals we support will live in a 10 square mile range centered in Flowery Branch, Georgia (Hall County)

Educational Preferences:

o Bachelors Degree

o One year professional experience in the support of individuals with disabilities Supervisory experience is highly desirable.
