Full-Time: 40 hours AM Shift JOB SUMMARY: The RN Unit Coordinator- MDS Coordinator/ Utilization Review/ Infection Preventionist reports directly to the Director of Nursing, Subacute Unit and is responsible for coordinating the interdisciplinary team in the timely completion, accurate documentation and transmittal of the Minimum Data Set, implementation of Infection Prevention/Control Activities and data tracking, and conducting/ managing patient Utilization Review. DUTIES AND RESPONSIBILITIES 1. Coordinates the MDS process for all residents in accordance with state and federal regulations. Appropriately codes patients diagnoses. 2. Keeps the resident care plan and assessment up to date. 3. Ensures that data is accurately entered into the computer and transmitted to CMS in accordance with the regulations. 4. Assumes the overall responsibility for the coordination of patient care planning and actively participates in the process. 5. Analyzes and monitors facility quality indicators derived from CMS database and notifies the Director and develop action plans. 6. Coordinates and implements Infection Control and Infection Prevention activities and principles on the Unit. (Completed CMS Infection Control Course.) 7. Coordinates Utilization Review activities, updates payers on patient progress. Works with Business Office to obtain authorizations, track and bill patient days. Assists with collections as needed. 8. Avoids and prevents injury to self and patients by applying the principles of body mechanics. Maintains a safe environment, functioning with an awareness and application of safety issues identified within the unit/facility, i.e., suction and oxygen immediately available when needed. 9. Recognizes signs of abuse and reports timely and appropriately to the Unit Abuse Coordinator. 10. Performs other duties as assigned or required.

Required Qualification: EDUCATION, EXPERIENCE, TRAINING 1. Must have current and valid license as a Registered Nurse (RN) issued by the State of California. 2. BCLS upon hire and maintain current. 3. Computer skills required. 4. Minimum of two years recent SNF experienced preferred. 5. Must complete all modules of the CDC Nursing Home Infection Preventionist Training Course and pass Final Exam within 90 days of hire. (Or must be able to provide evidence of having completed Course and passed exam prior to hire.)
