Hiring Range : $17.17-$22.32
The actual compensation for this position will be determined based on experience and other factors permitted by law.
Responsibilities:
- Establishes contact and rapport with patients and their families, and provides periodic updates to family members.
- Intermediates between the patient, family members, physicians, community organizations, schools and care providers, as needed to facilitate communication and resolve issues.
- Establishes follow up services, ensuring that all items on the patient care plan have been completed; collaborates with the next level of care including arranging for appointments as indicated.
- Addresses customer service issues with proper follow-up or referral, uses service recovery resources, and communicates issues and resolution to appropriate level.
Education :
- High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.
