St. Margaret's Center , an affiliate of Center for Disability Services, is a pediatric and young adult 94-bed skilled nursing facility, located in Albany, NY. Founded in 1883, St. Margaret's Center is a skilled nursing facility serving medically fragile infants, children, and young adults on a single dedicated campus. We specialize in providing cutting-edge, long-term, 24-hour intensive medical care in a nurturing and home-like setting.

The Clinical Care Coordinator is responsible for the overall organization of resident medical records and administrative duties as outlined on the assigned unit. This includes scheduling medical appointments/ transportation, keeping track of labs and diagnostic reports, and assisting with timekeeping and attendance for staff on assigned unit. Will function as a receptionist for assigned unit by answering the phone, as well as addressing the needs of the caller appropriately.
Requirements for a Clinical Care Coordinator:
  • High School Diploma or equivalent required; Associates Degree preferred, in field of Business or Medical Office Assisting, Business Administration, or related field
  • Minimum of 3 years of job related experience, specifically in: Medical Records Management, Business Office Administration, and/or Clinical Care Coordination
  • Knowledge of basic procedures in healthcare industry for medical records and advanced computer skills
  • Must have strong organizational skills and the ability to prioritize tasks
