The team Care Manager is responsible for conducting facility and community based comprehensive biological, psychological, social, level-of-care, and spiritual assessments; assessing and motivating the opioid population to address recovery needs impacted by areas of life domains; making appropriate referrals to community and treatment supports; and coordinates level of care transition needs. In addition, provides resources and education to both persons served and family members of persons served. JOB DUTIES AND RESPONSIBILITIES: Take the role in providing admission, assessment, and level-of-care placements within the Center of Excellence (COE). Create, review, and monitor COE client profiles to ensure they represent the most up-to- date information. Identify from assessments, the care management classification for the person served consisting of needing care management, complex care management, or transitional care management COE interventions. Identify COE persons served that have disconnected from recommended clinical care through electronic health record review with a minimum of a weekly review. Complete mobile assessments as requested that may be located at emergency rooms, medical offices, and the criminal justice system. Coordinate transportation needs to treatment facilities. Ensure warm handoffs with COE team and the St. Luke's Penn Foundation service delivery. Complete referrals and follow-up to all behavioral supports identified through assessment process. Gather and enter information into PA WITS system, including GPRA as necessary for county funded clients PHYSICAL AND SENSORY REQUIREMENTS: Must have ability to sit for 2 total hours per day, and up to 1 total consecutive hour. Must be able to stand for 8 total hours per day and up to 2 total consecutive hours. Must be able to walk 6 total hours per day and up to 10 total consecutive minutes. Must have the ability for hearing as it relates to normal conversation, high frequency and low frequency hearing sounds. Must have the ability for seeing as it relates to general vision, near vision, color vision and peripheral vision. EDUCATION: Bachelor’s Degree in the human services field required. REQUIRED: Must possess and maintain a valid driver’s license along with a driving record in compliance with St. Luke’s Penn Foundation policy. WORK SCHEDULE: Monday-Friday 1st shift, with flexibility for early evening hours as needed. On-call rotation as scheduled.
