St Johns Community Health is Hiring a Patient Care Manager Near Los Angeles, CA

Position Summary

The Patient Care Manager – San Bernardino - is responsible for non-medical case management activities for HIV and HCV positive and/or substance using patients experiencing homelessness or housing insecurity, and patients at risk for HIV. The coordinator is responsible for ensuring that all goals are being met and report on program progress. This is demonstrated through working with insurance carriers and Medi-Cal programs to ensure Prior Authorization and Treatment Authorization Requests are filled, working with other Programs staff to link patients to appropriate services and care, HIV testing and linkage to care, and assistance in creating promotional materials for use in mediums including, but not limited to, social media, email, and outreach events.

This position plays an essential role in patient-facing care and will be the “face” of the program and be comfortable with establishing rapport and demonstrate the ability to communicate with patients from marginalized and underserved population.

Qualifications/Licensure:  

  • High School Diploma required in related field (management, business, public health) 
  • Two years’ related experience within human services and/or public health; 
  • Experience in community organizing; 
  • Experience working in Teams; 
  • Experience working in underserved communities such as undocumented workers, unaccompanied minors, LGBT populations, communities of color, youth/adolescents, and those experiencing homelessness, substance abuse, and/or mental illness; and Bilingual English/Spanish (Required) 
  • Familiar with Medi-Cal, Healthy Families and Healthy Way LA 
  • EHR- Electronic Health Record (Required) 
  • Maintains personal appearance when interacting with patients, staff and physicians. 
  • Must be able to cope in a “fast pace” environment.

ESSENTIAL DUTIES AND RESPONSIBILITIES  

  • Review and complete prior authorization requests. 
  • Assist with promoting program services and schedule appointments for transgender patients. 
  • Establish and develop working relationships with clients. 
  • Initiate and conduct outreach to the target population using telephone, mail, email, and social media, and attending outreach events as directed.
  • Assist in the development of educational/promotional materials of the Transgender Health Program. 
  • Perform outreach activities at community events as business needs dictate.
  • Assist patients who require support and case management.
  • Conduct HIV testing and link patients to PRIME Program for care and retention.
  • Conduct surgery referral orientations and process surgery referrals as directed by medical providers.
  • Cross-train to support the program during THP staff absences/leaves and assist as necessary to maintain high levels of patient care.
  • Attend regularly scheduled and impromptu meetings and maintain communication with program team members and supervisor.
  • Assist in the production of required weekly, monthly, and/or quarterly data collection reports. 
  • Assist with program evaluations and updates to assigned program and services.
  • Work in collaboration with other departments when required.
  • Attend meetings, trainings, and other work-related events as needed; and 
  • Other duties may be assigned or may be modified as business needs dictate
