We are currently seeking a Full Time Registered Nurse (RN) Clinical Coordinator!

Responsibilities:

The RN Clinical Coordinator position is responsible for clinical management on the unit in coordination with Nursing Staff and management. The RN Clinical Coordinator will be responsible for performing ongoing rounds and RN clinical assessment on high risk patients. This position creates and reviews clinical documentation for the unit to support an effective clinical management support.

  • Review residents’ reports with Charge Nurse, discuss and note residents conditions, taking note of critical Residents, physician’s orders, floor coverage, and similar details.
  • Supervise rounds done on the floor. Help and provide guidance where needed.
  • Follow up re: need for bowel/med administration for residents on an individual basis. If concerns noted, follow-up with charge nurse.
  • Review treatment sheets for those scheduled for assigned shift. Treatments as scheduled-confirm documentation is consistently completed.
  • Follow-up as needed with residents re: significant changes prior to or on prior shift. Notify DON/ADON of concerns. Contact MDs when needed for change in resident’s clinical status or a need for orders based on resident complaints, lab results, etc.
  • Review resident care plans for appropriate resident goals, problems, approaches, and revisions based on nursing needs.
  • Ensure that nursing documentation is complete, accurate and completed within the appropriate time frames.
  • Participate in the development and implementation of policies, procedures and standards of resident care.
  • Monitors unit staff compliance with all facility and nursing department policies and procedures.
  • Plan for, coordinate, and facilitate resident admissions and transfers to respective nursing units.
  • Attend and participate in family conferences.
  • Coordinate referrals for consulting services.
  • Collaborate with physician and Nurse Practitioner regarding resident problems, daily medication needs and monthly renewals.
  • Facilitate communication among physicians, residents, families, and interdisciplinary team members.
  • Function as facility supervisor as scheduled.
  • Participate in Nurse Manager meetings as scheduled by the Director of Nursing.
  • Serve as a resident and family advocate and update families on change in status of residents.
  • Maintain the confidentiality of all resident care information and ensure that unit staffs respect all residents’ rights.
  • Respond to emergency situations within the facility according to Code and Disaster policies and procedures.
  • Participate as a member of nursing and/or facility committees.
  • Assist in preparing written and/or oral reports of the nursing service programs and activities to submit to such committee(s) as directed.


Qualifications:

  • Registered Nurse License with current registration in New York State
  • Graduate of an accredited School of Nursing, BSN preferred
  • At minimum, 3 to 5 years-related experience working in a health-care facility.
  • At minimum, 2 years of supervisory experience.
  • Strong verbal, written, computer and leadership skills required.
