The MDS Coordinator at St. Ann’s Home exemplifies the vision, the mission, and the vibrancy of the organization and leads the delivery of exceptional care to our residents.

Teamwork and collaboration are instrumental to the success of our communities, and in this role, you will work with the internal leadership team, including our Director of Nursing, and our external partners to oversee and guarantee that our resident's needs are not only met, but delivered with vibrancy.

Essential Functions :

The MDS Coordinator facilitates the completion of the Omnibus Reconciliation Act (OBRA), Prospective Payment System (PPS) assessments and Level of Care Determination (LOCD).

Will schedule the ARD’s (Assessment Reference Date) for Payment Assessments with the Rehab Director daily, weekly and as needed in a manner that accurately captures the CMG (Case Mix Group) or RUG (Resource Utilization Group) category.

Will schedule all OBRA ARD’s daily, monthly, and as needed. This position is also responsible for wound rounds within the facility.

Duties / Responsibilities : In this role, you will assist in coordinating nursing services with other resident services to ensure the continuity of the residents’ total regimen of care.

Manage the overall process for tracking all the Medicare documents to assure appropriate reimbursement for services provided.

Update and monitor the ICD-10 coding. Participate in developing, implementing, and periodically updating the resident’s written plan of care.

Coordinate and monitor the physician / provider services program, notify the Director when visits are not made promptly.

Coordination and follow-up of incident process, including root cause analysis and IDT documentation. Monitor long-term care residents related to functional declines and report changes in function to Physician / Interdisciplinary Team.

Administration : Responsible for the development of written comprehensive assessments (MDS, LOCD’s and CAA’s) of each resident.

Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.

Provide Case management / Insurance updates for residents that enter facility with Medicare Advantage, Bundled, and Private insurance plans.

Review nurses’ notes and assessments to ensure that they are informative and descriptive of the nursing care being provided, that they reflect the resident’s response to the care, and that such documentation meets CMS regulations.

Will ensure the transmission of Assessments are timely. Participate in developing, maintaining, and updating written policies and procedures in relation to MDS.

Communication : Coordinate improvement of and facilitate Medicare meeting. Will coordinate the completion of MDS sections according to facility assignments.

Follow up on concerns identified. Collaborate with therapy regarding restorative care services, programs, and activities in accordance with current rules, regulations, and guidelines that govern nursing care facilities.

Teamwork : Work collaboratively with the

Therapy Program

Manager to identify appropriate ARD dates for optimal reimbursement. Work collaboratively with Social Work and IDT in coordination of appropriate resident discharge dates.

Unparalleled Service : Uphold for team and self the highest standards of customer service to residents. Assist team as needed, to exceed resident’s expectations.

Flexibility : The ability to shift gears and implement new topics and processes quickly to address emerging resident needs.

The work schedule may require conformity to resident needs and outcomes.

Other duties as assigned to meet standards and organizational goals.

Education and Experience Requirements :

  • Must possess, as a minimum, a Registered Nursing Degree from an accredited college or university.
  • Must possess a current, unencumbered, active license to practice as an RN in this state.
  • Current CPR certification required.
  • Minimum of two years’ experience in a long-term care environment.
  • Ability to work in a fast-paced environment with multiple competing demands for time and attention.
  • Ability to lead, direct, and motivate others to innovate and excel.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.
  • Excellent verbal communication skills with the ability to interact with a diverse group of associates, residents, and family members.
  • Ability to apply reason, logic, and advanced problem-solving skills to resolve complex and / or sensitive issues.
  • Excellent written communication skills including proper grammar and professional writing.
  • Ability to read at a level to understand care notes and written directives of medical practitioners.
  • Strong computer skills and keyboarding skills with proficiency in Microsoft Word and Excel and the ability to quickly master new applications.
  • Positive attitude, enthusiasm and energy.

Working Conditions :

  • Involved with physicians, residents, personnel, visitors, government agencies / personnel, etc., under all conditions and circumstances.
  • Subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
  • Involved in community / civic health matters / projects as appropriate.
  • Attends and participates in continuing educational programs.
  • Subject to injury from falls, burns from equipment, odors, assaults from hostile residents, etc., throughout the work day, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
  • Subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
  • May be subject to the handling of and exposure to hazardous chemicals

Physical Environment :

  • Must be able to move intermittently throughout the workday.
  • Must be able to speak and write the English language in an understandable manner.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must be able to see, hear or use prosthetics that will enable these senses to function adequately to ensure that this position's requirements can be fully met.
  • Must function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
  • Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within St. Ann’s Home.
  • Must be able to push, pull, move, and / or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and / or carry such weight a minimum distance of 50 feet.
  • May be necessary to assist in the evacuation of residents during emergency situations.
