Position Summary:

Coordinate care with the patient, family, referring agency, and assumes responsibility for the coordination of care. Provides service in accordance with the Agency philosophy, standards and policies.

Essential Functions:

  • Responds to a request for initial assessment within 24 hours.
  • Develops a written plan of care for each patient, assessing the physical, psychco-social needs of the patient and documents the findings.
  • Assesses patient’s condition, initiates plan of care, re-evaluates and updates on a 60-day basis and as medical necessary
  • Assesses the execution of patient care plan on a weekly basis and address any gaps in care immediately
  • Educate the patient, family and caregivers on care plan.
  • Performs patient care responsibilities considering needs specific to the standard of care for patient’s age.
  • Demonstrates knowledge of medications and their correct administration, based patient’s clinical position.

§Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

§Re-evaluates the need for continued care on an ongoing basis

§Plans for discharge from the Agency with the patient and caregivers. Makes referrals for alternative level of care if necessary.

§Promotes patient/family/caregiver education using various verbal and written communication techniques that take into account the patient’s/family’s cultural, ethnic and/or personal needs or preferences.

§Supervises Home Health Aides on an as needed basis

§Initiates needed referrals to other disciplines and/or community resources.

§Obtains needed supplies and equipment to provide care.

§Knowledgeable of current federal, state and agency regulations concerning patient care.

§Adheres to all Agency, payroll, billing and documentation policies and procedures

§Participates in Manager on Duty Program (see below)

§Assist Director of Operations and staffing coordinator with the hiring and orienting and disciplining of all clinical. 

Manager on Duty Program:

  • All management staff will be on call on a rotating basis 1 week at a time. On Call will include after 5pm Monday through Friday, all day weekends and holidays
  • Answers incoming calls promptly and efficiently. An immediate response is required.
  • Promptly communicates issues/concerns/actions to Director or RN Supervisor
  • Completes and maintains on call report on a daily basis
  • Reviews on call report with appropriate personnel weekly/daily as needed

Non Essential Functions:

  • Attends all required training, in-services, and staff meetings.
  • Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, the achievement of safe working practices, by practicing proper infection control measures, and understanding the use of Material Safety Data Sheets (MSDS).
  • Maintains a positive and professional demeanor toward residents, visitors, families, and co-workers.
  • Adheres to all policies and procedures of Bedrock at Home NJ, LLC
  • Performs all other duties as assigned.

Experience And Education

§Graduate of an accredited school of nursing.

§Registered Nurse with an active and unrestricted license in state of employment 

§Two years experience as a Registered Nurse in a clinical care setting preferred. Home Health preferred.

§Current CPR card

§Valid state driver’s license and reliable automobile.

§Proof of current automobile insurance.

Time spent in physical exertion: Standing (1/3- 2/3), walking (over 2/3), sitting (under 1/3), using hands to finger, handle or feel (over 2/3), reach with hands and arms (over 2/3), climb and balance (1/3-2/3), stoop, kneel, crawl (2/3), talk or hear (over 2/3), taste or smell (under 1/3). Lifting up to 10 pounds (over 2/3), Up to 25 pounds (1/3); Up to 50 pounds (under 1/3); up to 100 pounds (under 1/3)
