Role : Clinical Case Manager, Permanent Supportive Housing - Westside Reports to: Program Manager Site Name : Cloverfield Department : Housing Program : 1603-Permanent Housing Services: Housing Retention Location : Cloverfield Blvd., Santa Monica, CA 90404 Status : Full time, non-exempt (hourly), non-management Benefits : Medical, Dental, Vision, Retirement Planning (403b), EAP, etc.
SUMMARY In partnership with the Department of Health (DHS) and the Office of Diversion and Re-entry (ODR) Team the Clinical Case Manager will provide case management and clinical services to clients recently released from jail who are experiencing homelessness. The Clinical Case Manager will meet clients in jail, assist with coordinating their release and help them attain permanent supportive housing in the community. The ODR Clinical Case Manager will work with clients to provide intensive field-based case management and clinical services while employing trauma-informed care and harm reduction approach to a diverse group of clients with complex trauma-facing medical, mental health, and substance use barriers. Clients remain in the program for as long as they require subsidized housing. For many individuals, this may mean their entire lifetime.
ESSENTIAL DUTIES & RESPONSIBILITIES
1. Work with a diverse and marginalized client caseload of 15 justice-involved clients experiencing many barriers including homelessness, complex trauma, medical, mental health and substance use
2. Assess mental health service needs of clients, provide therapeutic support and link to Full-Service Partnership (FSP) services
3. Provide crisis intervention and safety assessments to clients on caseload
4. Participate in court hearings and submit court progress reports
5. Weekly coordination with client's multi-disciplinary team which might include psychiatrists, judge, probation officer, FSP team and DHS partners
6. Link clients to appropriate medical, mental health, substance abuse, and vocational resources
7. Assist clients in obtaining all benefits to which they are entitled
8. Opportunities to create and facilitate support groups focusing on housing stability, community integration and improvement of mental health
9. Ensure services to clients are reflective of the "Whatever It Takes" model, which emphasizes customer choice, harm reduction, trauma-informed care and psycho-social rehabilitation
10. Ensure that each client on caseload has an up-to-date and comprehensive biopsychosocial assessment
10. Collaborate with each client to develop creative goal plans aimed at improving overall well-being and housing stability; review and update quarterly and upon completion of goals
11. Develop and maintain a complete, accurate, and current client file with all required documents and data in agency records and electronic databases
12. Document all client contacts in PIP format (Purpose, Intervention, Plan) and in accordance with Housing Department productivity expectations
13. Maintain confidentiality of client files per HIPAA and all applicable guidelines
14. Transport clients in your vehicle to various agencies and healthcare providers in the greater Los Angeles area to increase community support services and community reintegration
15. Assist in the screening, assessment, and enrollment of clients, including orientation to program policies, resources, and goals
16. Maintain current, thorough knowledge of community resources and utilize them to provide comprehensive, wrap-around services to clients
17. Work cooperatively and cohesively with other clients of the staff team, including participation in weekly staff meetings and staff training
18. Participate in weekly individual and group clinical supervision
QUALIFICATIONS 1. MSW, MFT or LPC who is BBS eligible (supervision hours are available for this position)
2. Registered with the board of behavioral sciences or ability to do so within the first 2 months of employment
3. Able to pass a background check for jail clearance
4. Demonstrated knowledge of clinical case management
5. At least one year of experience working with clients experiencing homelessness and with mental illness, chronic health issues, and substance use disorders.
6. Knowledge of community resources
7. Able to work in a high-tolerance, harm-reduction model with individuals who have multiple barriers
8. Strong clinical, team-building, charting, and non-violent crisis intervention skills
9. Computer literate; able to effectively use a computerized database for client file management, with basic skills in Microsoft Word, Outlook, and Excel programs
10. Detail oriented with strong time management, organizational, written, verbal, critical thinking and interpersonal skills
11. Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record
12. Able to transport clients in your vehicle 13. Minimum 30 WPM typing speed preferred
WORK ENVIRONMENT
1. Combination of field and office environment
2. Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
3. On occasion walk or drive to different local sites
4. Will necessitate working in busy and loud environments
5. Will be exposed to elements like cold, heat, dust, noise, and odor
6. May need to bend, stoop, twist, and sit throughout the day
