Job Description:

MDS Coordinator oversee and facilitate the completion of resident assessments at nursing facility Throughout the assessment and while planning care, they ensure compliance with Federal and State regulations.

Duties include:

• RUG and Case Mix Index scores
• Responsible for working with Medicare Advantage plans for the continuation of resident’s benefits
• Responsible for completing the MDS process and Care Plans.
• Responsible for meeting Federal/State guidelines regarding MDS/Care Plan process and time frame.
• Responsible for the ICD-9 coding.
• Attend weekly therapy update meetings.
• Complete assignment in accordance with facility policies and procedures, resident care plan state and federal guidelines.
• Print and communicate on MDS related reports.
• Provide direct nursing care as assigned.
• Update Census.
• Review medical records for completeness; ensure all required information is included, note deficiencies, and refer to appropriate area for follow-up.
• Be on a nursing on call rotation
• Other duties as assigned

Job Requirements/Qualifications:

• Licensed as a Practical Nurse or Registered Nurse
• Up to Three years’ experience in a Nursing home setting
• Knowledge of nursing & medical practices & procedures
• Great communicator and team player a must.
• Must be honest and have high level of integrity!
