Job Summary:
Individuals will partner with primary care teams to assist PCPs (Primary Care Providers) in managing their panels of patients. Job tasks include triage, prescription refills, care management, patient education and medical visits. In this role, they would enhance their understanding of triage, diagnoses, treatment, and care coordination. This work would enhance the knowledge and capacity of the new graduate NP and would evolve in their independent management of simple to increasingly complex direct care. Work would include didactic instruction and close supervision and precepting by experienced PCPs as the NP grows to become integral to the primary care team.
Employees in this position who show excellence in their skills could transition to a full-time PCP role at the end of 1-3 years of employment.
Qualifications: Master of Science in Nursing or Doctor of Nursing Practice degree from an accredited school of graduate nursing; active license for Registered nurse and Nurse Practitioner in the Commonwealth of Massachusetts.
