Responsibilities:

• Complete initial assessments

• Implement care plans

• Assist with discharge planning

• Document patient progress and coordinate family meetings

• Ensure regulatory compliance

• Complete MDS sections as assigned, ensure all areas are addressed in the care plan as necessary.

Requirements:

• A bachelor's degree in social work or a closely related field

• A master's degree a plus

• State licensure preferred (CSW, ACSW, LSW, LCSW)

• Interpersonal, critical thinking, and organizational skills
