We are seeking a highly skilled and compassionate Home Health Assessment Nurse to join our team. In this role, you will be responsible for providing comprehensive nursing assessments, developing personalized care plans, and coordinating with multidisciplinary healthcare teams to ensure high-quality home health services for our clients.
Responsibilities
1. Conduct thorough in-home assessments of clients’ physical, psychological, and social needs.
2. Develop, implement, and evaluate individualized care plans in collaboration with clients, families, and healthcare team members.
3. Monitor clients’ progress, identify potential health issues, and adjust care plans accordingly.
4. Educate clients and their families on disease prevention, health promotion, and self-care strategies.
5. Coordinate care services with physicians, therapists, and other healthcare professionals to ensure optimal client outcomes.
6. Maintain accurate documentation of assessments, interventions, and client responses in accordance with regulatory guidelines.
7. Stay current with the latest evidence-based practices, research, and technological advancements in home health care.
8. Participate in quality improvement initiatives and contribute to the overall success of the home health agency.
Qualifications:
1. Registered Nurse (RN) with an active, unencumbered nursing license in the state of Illinois.
2. A minimum of 2 years of experience in a home health or community health setting.
3. Strong knowledge of nursing principles, practices, and techniques.
4. Proficient in conducting comprehensive health assessments and developing effective care plans.
5. Excellent interpersonal, communication, and organizational skills.
6. Ability to work independently and collaboratively within a multidisciplinary team.
7. Familiarity with electronic health records and documentation systems (Axxess).
8. Current Basic Life Support (BLS) and CPR certification.
9. Valid driver’s license and reliable transportation for home visits.
Preferred:
1. Certification in Home Health Nursing (e.g., RN-BC, HHCN)
2. Bachelor’s degree in Nursing (BSN)
