Needed for a full-time, M-F, temporary or temp-to-perm position serving the adult population in a Chicago-area hospital. Immediate need for a minimum eight-week assignment. Social workers are responsible for collaborating with patients, case managers, physicians, and interdisciplinary teams for discharge planning. Also responsible for assessments, psychoeducation, referral and linkage, counseling, and crisis intervention for assigned patients.
Services are provided in the inpatient and outpatient settings.
