Job Title: Associate Clinical Social Worker- ACSW - Community Counseling- At-Risk Youth

Examples of Essential Duties:

Evaluate patients’ psycho-social needs;

Evaluate patients' family/social situation to support a treatment plan;

Participate in on-going counseling;

Utilize community resources to assist patients' psycho-social needs;

Document history, care plans and prepare reports as needed.

Education Requirement:

Graduation from an accredited university or college with a Master Degree in Social Work.

Qualifications:

Current California Certification as an Associate Clinical Social Worker (ACSW);

Current AHA-approved CPR card
