POSITION SUMMARY:
The Home Health Aide is tasked with assisting the residents in their daily living to support their overall mental, physical, emotional, and spiritual health in a home environment. This includes but is not limited to support with personal hygiene, dressing, eating, and mobility for their well-being. All assignments are performed under the supervision of the RN or the LPN.
QUALIFICATIONS:
Current Home Health Aide Certification, current Certified Nursing Assistant certificate, or current Personal Care Aide certificate issued by a Home Health Care School or by an New York State Agency.
Good oral and written communication skills
Commitment to participate in continuing professional improvement.
Ability to show compassion, respect and understanding.
Able to communicate effectively across all levels of Residents, staff, and management.
Ability to follow HIPPA guidelines and confidentiality.
Strong listening skills
Ability to work together as a team.
Experience with MatrixCare EMR a plus
Must be fully COVID 19 vaccinated.
RESPONSIBILITIES (include, but is not limited to):
Provide direct resident care in the activities of daily living including grooming, dressing, mobility, transfer, eating, bathing, and toileting.
Clean and care for beside equipment and the Sisters total environment.
Report all incidents and accidents immediately to the floor nurse.
Documenting of ADL's, by tracking, monitoring, and reporting activity into EMR system.
Assist with personal laundry, when necessary.
Promote self-care activities.
Provide continued observation and gives assistance when needed.
Maintain clean workspaces which includes the Sisters room, utility rooms, clean linen closets, laundry areas and supply carts.
Assist in Emergencies as needed.
Attendance at department meetings.
Know and carry out the mission of the Sisters of St. Joseph to build unity among all created beings and promote a work environment that is characterized by respect, trust, integrity, ethical behavior, fair treatment of all, open communications, and personal accountability.
PHYSICAL DEMANDS:
While performing the duties of this job, the employee is frequently required to exhibit strength and endurance to stand, walk and sit.
Must be able to lift up to 125 lbs., carrying, pushing and pulling.
Frequently exhibiting balance and coordination crouching, crawling, stooping, kneeling, manual dexterity, and tactile sensitivity.
Frequently communicating by talking, hearing, and seeing.
