Sisters of Mercy of the Americas Incorporated is Hiring a Life Care Coordinator Near Farmington, MI

JOB PURPOSE:

The position is responsible for providing direct services, providing clinical case management and providing an emergency response for the Sisters of our religious community. The Life Care Coordinator works collaboratively with individual sisters, identified companions, and the Sister Life Minister (SLM) to assess, plan, facilitate, provide and advocate for options and services to meet the health and wellness needs of the Sister. Life Care Coordinators assist the Sister in achieving wellness and autonomy through advocacy, communication, education, identification of service resources, and service facilitation.

This position serves Sisters who reside in Farmington Hills, Michigan and the surrounding areas. Frequent travel across this area is expected. Infrequently, overnight travel may be required.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Provides direct care, clinical case management, support and resource linkage for the Sister to ensure health and safety needs are met.

2. Understands, assists and supports enrollment and coordination of government healthcare services, equipment, and other benefits.

3. Develops a working relationship with each sister and interacts regularly with each sister.

4. Assists the Sister Life Minister and the Sister in transitioning to a higher level of care living situation.

5. Provides clinical oversight and consultation for a Sister living in care facility, and those living independently.

6. Interacts regularly and effectively with community care/service providers to ensure a coordinated health care approach to assisting a sister.

7. Interacts regularly and effectively with the Sisters Sister Life Minister and DPOA for Healthcare.

8. Schedules and conducts wellness profiles/assessments in accordance with established procedures.

9. Maintains a record/documentation of clinical contacts and services provided.

10. Provides education, resource linkages, and ongoing support to the Sister.

11. Maintains privacy, confidentiality, and protects sensitive data at all times.

12. Coordinates with the Personal Support Assistant to ensure scheduling of transportation and personal needs of the Sisters is provided on a consistent basis to ensure the resident’s total regimen of care is maintained.

13. Meets regularly with other Health & Wellness personnel as scheduled.

14. Participates in scheduled departmental meetings and educational classes.

15. Incorporates the values and mission of Sisters of Mercy in the carrying out of responsibilities.

QUALIFICATIONS:

Education, Competencies and Experience

Required

1. Nursing degree (RN) or Master’s in Social Work..

2. Professional license in nursing or social work.

3. Minimum of 2 years of experience with older adults.

4. Ability and willingness to work flexible hours.

5. Ability and willingness to travel by both plane and car when needed.

6. Possess a valid State Driver's License.

Pay: $32.00 - $38.00 per hour
