General Summary of Duties:

The Director of Case Management is responsible for overseeing the implementation of the hospital's case management program. This includes care facilitation, utilization management, case management, and discharge planning.

Responsibilities:

• Provide leadership, education, and supervision for Case Managers and Social Workers.
• Monitor documentation to ensure regulatory compliance.
• Collaborate with CFO and Quality Department for quality improvement programs and data trending.
• Maintain proficiency in case management and utilization review to cover patient caseloads.
• Communicate with physicians regarding patient needs and assist in developing treatment plans.
• Oversee personnel actions including hiring, performance appraisals, scheduling, and payroll.
• Facilitate Multidisciplinary Rounds for holistic patient care.
• Participate in discharge planning and provide necessary education and resources.
• Actively participate in Utilization Review and Revenue Recycle Committees.
• Promote efficient utilization of clinical resources.
• Ensure compliance with applicable regulations and standards.

Knowledge, Skills & Abilities:

• Understanding of payer requirements and discharge planning regulations.
• Knowledge of Medicare, managed care, and healthcare continuum.
• Ability to work collaboratively with healthcare professionals.
• Familiarity with performance improvement concepts.
• Self-motivated with strong communication and assertiveness skills.
• Effective working relationships with physicians.

Education:

• Graduate of a Registered Nursing program.
• Bachelor of Science in Nursing degree preferred.

Experience:

• Minimum of two years of Case Management experience.
• Two to three years of previous management experience preferred, with a minimum of two years in hospital-based nursing.

Certificate/License:

• Iowa Mandatory Reporter – Child and Dependent Adult Abuse Certificates.
• Current RN license in the state of Iowa or a multistate license allowing work in Iowa.
