Job Duties:
- Assesses/re-assesses hospice patients.
- Performs nursing services and treatments per physicians orders.
- Communicates with Hospice physician team.
- Acts as case manager to coordinate all aspects of patient care.
- Participates in the on call rotation.
- Supervises hospice aides and other staff members as appropriate.
Qualifications: Must be licensed as a RN in the state of Ohio; must have at least 2 years experience as a nurse, Hospice experience strongly preferred; Leadership qualities are strongly preferred.
