Sholom Community Alliance is Hiring a Registered Nurse - Home Care Near Paul, MN

SUMMARY

The Registered Nurse (RN) Homecare ensures that the health and safety of all Homecare clients are provided through regular assessments, service plan development and supervision of care as provided by Home Care program staff and assists in marketing the Homecare program.

PRIMARY RESPONSIBILITIES

1. Provide direct care to residents under individual plans of care, administering care and medications per doctor’s orders. *

a. Act as an advocate to maintain client privacy and dignity during cares. Administer prescribed medications and treatments per doctor’s orders. *

b. Monitor resident vitals, completing resident assessments, communicate any identified changes to physician and family members. *

2. Collaboratewith the IDT (interdisciplinary team) to ensure adequate and appropriate flow of services*

3. Complete the comprehensive assessment for pre-admissions, new admissions, hospital transfers and discharges *

4. Complete comprehensive assessment of residents with acute changes of condition. * 

5. Attend resident Care Conferences as needed. *

6. Supervise, oversee and lead unlicensed staff, ensuring Minnesota Homecare Rule and Statute requirements are met. *

a. Communicate expectations and ensure that unit personnel are carrying out their assignment. *

b. Organize and provide work direction to unit staff. *

c. Ensure staffing needs are met in collaboration with the Staffing Coordinator. *

d. Serve as a clinical resource to staff regarding care of residents. Provide in-service education as requested. *

7. Maintain and complete all required documentation and charting in the electronic medical record or on forms in an accurate and timely manner according to policies and procedures. *

a. Accurately transcribe Physician’s Orders as per facility protocol and forward necessary information to other departments. *

8. Update Service plans and service contracts as needed*

9. Ensure and follow billing practices and procedures*

10. Support various other Nursing responsibilities as needed.

JOB SPECIFICATIONS

Education, Experience and Credentials

· Must be licensed as a Registered Nurse in the State of Minnesota and must maintain licensure.

· Two or more years of previous related experience in a similar position and in a similar care environment preferred.

· Current certification in CPR preferred; upon hire if not certified, must obtain no later than the completion of on the floor orientation

· Able to understand and follow facility and department safety and emergency procedures.

· Strong computer skills, including Microsoft Word, Excel, Outlook, Internet, HRIS, and other applications. Must be skilled in database management and record keeping.

· Understand and maintain confidential nature of organization and client information in accordance to HIPAA regulations and corporate compliance standards, including appropriate access of data housed within the Information Technology infrastructure. 

PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB

Incumbent must be able to:

· Perform heavy work, lifting up to 50 pounds, and frequently lifting/carrying of objects weighing up to 20 pounds as needed.

· Push/pull residents in wheelchairs up to 50 pounds a distance of several feet as needed.

· Twist, turn, bend, stoop and reach frequently in the performance of client assistance responsibilities.

· Communicate clearly and effectively in the English language both orally and in writing to all levels of people inside and outside the organization, including possessing good speaking skills, fluency and understandability.

· Able to effectively use office equipment to review and create information.

· Work at a desk for periods and perform work sitting at a computer. 

· Frequently stand and walk to move around office/building repeatedly throughout the day.

· Able to work a flexible work schedule, including overtime, nights and weekends as required including attending events/meetings as needed.

· Able to obtain reliable transportation

TOOLS AND EQUIPMENT USED

· Use thermometer, stethoscope, blood pressure cuff, wheelchair, patient lifts, and related medical equipment.

· Use telephone, computer and standard office equipment. 

WORK ENVIRONMENT

The work environment is indoors. The incumbent must be flexible about switching positions quickly in order to respond to the needs of residents, in responding to emergencies. May be at risk of exposure to blood, bodily fluids, and other potentially infectious material.
