Responsibilities:
- Coordinate and oversee the MDS (Minimum Data Set) process for patients/residents in a healthcare facility
- Ensure accurate and timely completion of MDS assessments and documentation
- Collaborate with interdisciplinary team members to gather necessary information for MDS assessments
- Conduct comprehensive assessments of patients/residents to determine their care needs
- Develop individualized care plans based on assessment findings and resident goals
- Monitor and evaluate the effectiveness of care plans and make necessary adjustments
- Provide education and training to staff regarding MDS requirements and processes
- Ensure compliance with state and federal regulations related to MDS assessments and documentation

Experience:
- Previous experience in a healthcare setting, preferably in a long-term care facility or skilled nursing facility

-Experience in MDS certification and training
- Knowledge of discharge planning, coding, ostomy care is highly desirable
- Familiarity with electronic health record systems and MDS software is preferred
- Strong attention to detail and excellent organizational skills
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills

Pay: $40.00 - $41.00 per hour
